I would like to make a ration card for my family, how do I proceed?

My husband and I have our names included in the rationcard of my FIL, but since we are moving separate we need to make a new card in which our children's name could also be included. We stay in R.T.Nagar. Which is the nearest Food and Civil supplies dept in that area?
